Medicare and ACA coverage: start with eligibility
Updated September 20, 2026. Medicare and Marketplace coverage under the Affordable Care Act (ACA) serve different situations. They are not simply two plans that everyone can choose between.
Who may qualify?
Medicare generally serves people age 65 or older and some younger people with qualifying disabilities, end-stage renal disease or ALS. Other eligibility requirements apply. ACA Marketplace eligibility depends on residency and other requirements; it is not defined only by being under age 65. Eligibility for financial assistance is a separate question and depends on household circumstances and applicable rules.
What should you compare?
Review monthly premiums, deductibles, copayments, coinsurance, covered prescriptions and the doctors and hospitals available under each option. Medicare includes different coverage paths; a Medicare Advantage plan has its own network, costs and rules. No option is automatically the cheapest for everyone.
Before changing coverage
Check your enrollment period and how new coverage affects existing insurance. Do not cancel a current policy until you understand when replacement coverage starts. Bring your current plan information, a list of prescriptions and your preferred doctors to the consultation.
